Health on Track
Who is this for?
The Health on Track program is open to people who have been 
diagnosed with diabetes who live
in the City of Greater Bendigo.
 
What are the benefits?
Diabetes is a complex health condition. We will work with you to help 
you manage your diabetes. Our Health on Track team can help you to 
navigate the system and provide the services you need to live well with diabetes.
 
How do I join?
You can be referred to the Health on Track team by your health care provider 
or you can contact Bendigo Community Health Services to make an appointment.
 
What happens when I join?
After you have been referred, you will be offered an appointment with the 
care co-ordinator who will assess your needs to organise access to services with you.
 
Cost?
Where possible, Health on Track services are provided at no costs. Some 
services have a minimal charge.
 
With our team of health professionals you can find out:

·                     How to monitor your blood sugar levels
·                     Get an assessment and help for your feet
·                     How to get more energy
·                     Get access to supervised exercise
·                     How you can manage your diabetes and prevent problems
·                     And much more

be arranged if required.
